Survey on obesity surgery

I'm currently a final year medical student writing a dissertation on NHS funding for bariatric surgery. There are many people like myself who have been refused life saving bariatric surgery on the NHS due to a lack of funding into this area of medicine forcing thousands of people to pay for it privately.
As part of my dissertation I have created an online survey to get people's views on this. The results of the survey will help me formulate a case for increasing the treatment available on the NHS.
